. Allogeneic mesenchymal stem cell transplantation in refractory ulcerative colitis: a case report. Mesenchymal stem cell transplantation (MSCT) has become a promising tool in the treatment of refractory ulcerative colitis. This technique involves infusion of intravenous hematopoietic stem cell transplantation from a HLA-matched sibling with graft-versus-host disease (GVHD) prophylaxis. Herein, we report a patient with refractory ulcerative colitis who failed to respond to mesalazine and corticosteroid treatment but who recovered after MSCT. A 34-year-old man with steroid-resistant ulcerative colitis presented with diarrhea, abdominal pain, bloody stool, and mucosal pallor, and was admitted to the Gastroenterology Department. Endoscopy showed severe mucosal edema and ulcers on the transverse and rectal colon mucosa. Ulcerative colitis (UC) was diagnosed based on endoscopic and histopathological findings. Unfortunately, the patient continued to have no response to corticosteroid and mesalazine treatments; therefore, he underwent MSCT from a 10/10 HLA-matched sibling. After three months, he recovered from diarrhea, abdominal pain, and bloody stool. The patient was successfully maintained on corticosteroids and mesalazine, and did not relapse during a follow-up of 13 months. Transplantation of allogeneic mesenchymal stem cells from HLA-matched siblings could be considered as a new therapeutic approach for steroid-resistant UC and as a potential treatment option for other diseases.15 Minutes to Midnight (2011) In 1776, the United States proclaims its independence from Britain.
